Community Support Coordinator
The Compass Program is a community-based program that supports people with disabilities to explore and experience life in their communities and pursue goals that enrich their lives. The CSC provides compassionate leadership, management and vision necessary to ensure the daily operations of the Compass Program. This person works closely with Community Direct Support Professionals (CDSP) to support participants of the Compass Program in the four key areas of: Growth, Relationships, Inclusion, and Choice.
The CSC ensures delivery of quality supports and services in a manner that reflects the philosophy and mission of Community Services, Inc. (CSI). Additionally, the CSC has the responsibility to accomplish strategic goals and objectives, including, but not limited to: adherence to regulatory requirements, policies, standard operating procedures, and best practices as defined by the organization.
Essential Functions:
- Demonstrates enthusiasm to build awareness, create relationships, engage stakeholders and instill excitement about the Compass Program and CSI’s mission.
- Participate in the development and implementation of the Individual Support Plan (ISP), and related documents that is reflective of the current goals and needs of each person.
- Champions the four Compass Points of Growth, Relationships, Inclusion and Choice.
- Provides supervisory oversight to CDSPs including: interviewing, hiring, scheduling, coaching, mentoring and training/development, performance evaluations, and progressive discipline as needed.
- Serves as positive role model for CDSPs, people receiving services, peers, and community members.
- Develop and maintain positive professional relationships with all stakeholders including: individual support teams, legal representatives, family members, regulatory agencies, and community partners.
- Ensures the health, safety and welfare of people served in the Compass Program by implementing identified supports in accordance with the ISP.
- Responsible to prepare for, participate in, and address deficiencies discovered during, State, Federal, Local or internal regulatory reviews.
- Ensure that all program activities operate consistently and ethically within the mission and values of CSI.
- Must be available during scheduled hours as defined by the Compass program.
- Deliver on goals as defined by CSI and the Compass program participants.
- Other duties as assigned.
- A minimum of two recent years’ experience supporting people with disabilities in a leadership capacity.
- Bachelor’s degree or equivalent life/work experience in combination with college level credits and training. Experience with Supported Employment, Day-Support Services or Therapeutic Recreation is preferred.
- Familiarity with person-centered planning, support plan development, and knowledge of state regulations (Oregon Administrative Rules) is required.
- Must be proficient with Microsoft Office Suite, office technologies, and electronic documentation systems such as Therap, payroll management, electronic learning management, and other similar systems.
- Excellent leadership, problem-solving, logistical coordination and communication skills.
- Ability to complete training as identified by the organization.
- Must be able to pass pre-employment drug screen.
- Must have a cleared and approved background check.
- Must have a current Washington or Oregon Driver’s License and acceptable driving record.
Physical Demands
- Ability to use proper techniques to move, transport, or transfer people receiving services.
- Ability to use adaptive medical and mobility equipment in a safe manner.
- Ability to travel within the assigned geographic area.
- Ability to sit and/or stand up to 8 hours per day.
- Ability to lift up to 50 pounds.
