Hospice Team Director

PURPOSE OF THIS POSITION
The Team Director manages and leads the delivery of high quality, cost effective end-of-life care through effective and efficient use of people and other resources. The Team Director oversees the total operation of the office; plans, organizes, directs, and controls human and fiscal/material resources in order to assure patients and families receive the highest quality care possible from available resources. Functions independently, with extensive autonomy within the constraints of policy and procedure, and as outlined by Clinical Director. Direction and guidance are available from Clinical Director, Nurse Consultant, Electronic Applications Manager and Educator, peer Team Directors, and corporate staff.
ESSENTIAL FUNCTION
QUALITY OF SERVICE
- Oversees provision of services, intervening as necessary to provide on a consistent basis, high quality, cost effective end-of-life care by staff, volunteers and contractors. Assists in developing and implementing Clinical Team initiatives including, but not limited to, clinical policy and practice, and clinical documentation.
- Demonstrates a strong commitment to quality assessment/performance improvement. Reviews programs and initiates quality improvement review and intervention. Guides staff and volunteers through change, manages transition.
- Works with corporate staff to comply with state, federal and local laws and regulations pertaining to provision of end-of-life services. Responsible for maintaining professional standards of all disciplines and following Care Initiatives Hospice policies and practices including, but not limited to, patient rights, confidentiality, clinical policy and practice, clinical documentation, standards of care, and human resource, financial and information management.
MANAGEMENT OF PEOPLE
- Supervises staff, volunteers and contractors providing services. Responsible for hiring, developing, coaching, and retaining competent staff by establishing and maintaining a professional work environment utilizing the interdisciplinary team approach. Accountable for competency of staff, conducting timely, written performance appraisals, and satisfactory employee opinion survey scores.
MANAGEMENT OF FINANCIAL RESOURCES
- Financial resources of the team are prudently managed without compromising care or long-term goals of team and Care Initiatives Hospice. Seeks ways to reduce costs to team and Care Initiatives Hospice. Accountable for planning, implementing and monitoring team budgets.
LEADERSHIP/MISSION FOCUSED
- Demonstrates leadership on a consistent basis. Participates in community and professional activities, as well as Clinical Team and Care Initiatives Hospice committees. Demonstrates ability to positively influence people, activities, and factors over which the individual has no direct authority or control.
- Works with Clinical Director to create, implement and evaluate Care Initiatives Hospice, Clinical Team and team strategic plans. Established clear and creative goals for team and staff that support the mission of Care Initiatives Hospice.
QUALIFICATIONS
Education/Certification/License
- Bachelor degree in related field preferred; Master degree preferred.
- Current, valid Iowa registered nurse license.
- Current, valid CPR certification.
- Current Iowa driver’s license and automobile liability insurance.
- Hospice and Palliative Nurse certification within two (2) years of employment.
Knowledge
- Knowledge of principles and practices for patient care disciplines delivering end-of-life care.
- Knowledge of federal and state regulations governing operation.
- Knowledge of quality assessment/performance improvement environments and techniques.
Experience
- At least one (1) year in a leadership position
- Demonstrated effective supervisory, communication, and interpersonal skills.
- Demonstrated fiscal and human resource management skills.
- Demonstrated ability to handle multiple priorities in an organized manner, make timely decisions, and exercise sound independent judgment.
Skills/Abilities
- High level of literacy in written and spoken English.
- Leadership and supervisory skills including strong human relations skills.
- Proficient interpersonal relationships and communication skills.
- Ability to follow written and oral instructions.
- Complete assignments timely, completely and accurately.
